A discharge summary (epicrisis report) summarizes the procedures performed during a patient's hospital stay, the diagnosis and the treatment plan. It is used for overseas treatment applications, requests for a second opinion and insurance files.

Diagnostic codes (ICD), drug names and dosage information are critical in this document; incorrect translation can have direct clinical consequences. The translation is performed by a translator experienced in the field and undergoes a second review for consistent terminology.

Certification requirements

Is notarisation required?

Notarization may be required for insurance and official applications; a sworn translation is generally sufficient for hospital applications.

Is an apostille required?

It may be required for official procedures. The report is first certified by the Provincial Health Directorate or an authorized chief physician's office; an apostille is then obtained on that certification from a provincial or district governor's office.
