A single-physician health status report documents a person's health at the time of examination under one physician's signature. Although it is a short document, three aspects are carefully preserved in the translation: its nature as a health status report (the statement that the person is fit for a specific job or activity), the physician's specialty and stamp and signature details, and the date of issue. Foreign institutions usually require a recently issued report; the translation schedule should be planned accordingly.

Is notarisation required?

Is an apostille required?

It is rarely required; if needed, the medical document chain applies (provincial/district governor's office after Provincial Health Directorate certification).

Can an e-Nabız printout replace a report?

This depends on the institution; many foreign institutions require a physician-signed report. As e-Devlet and e-Nabız documents can be verified by barcode, some institutions accept them; check the application list.

How is the expression "sağlam" translated into the target language?

Using established wording appropriate to the report's purpose: for example, "fit for work" for employment or "fit to attend school" for school. The file in which the document will be used determines the wording, so be sure to specify its intended use.
